A waiver form is needed for children attending Golden Stars to ensure safety and to acknowledge that parents accept responsibility for their child’s participation in all activities.

3. Cancellations
Refunds may be issued if cancelled before the program start date (subject to administrative fees). No refunds after the program begins or for missed days.
4. Assumption of Risk
Participation involves inherent risks. A signed waiver may be required before attendance.
8. Governing Law
These terms are governed by the laws of British Columbia, Canada.
6. Intellectual Property
All website content is owned by Golden Stars Club and may not be copied or reused without permission.
7. Liability
To the extent permitted by law, Golden Stars Club is not liable for indirect or consequential damages. Liability is limited to the amount paid for the program.
5. Conduct
We reserve the right to remove participants for unsafe or disruptive behaviour without refund.
Program Participation
By completing a booking or purchase, I give permission for my child to participate in activities offered by Golden Star. These activities may include indoor play, games, arts and crafts, workshops and group activities.
Assumption of Risk
I understand that children’s activities involve normal risks, including possible injury. I knowingly and voluntarily accept all such risks on behalf of my child.
Release of Liability
I agree to release and hold harmless Golden Star, including its owners, staff, instructors, volunteers, and representatives, from any claims or liability arising from my child’s participation, except in cases of gross negligence or willful misconduct.
Medical Authorization
In the event of an emergency, I authorize Golden Star's staff to seek medical treatment for my child if they are unable to reach me. I understand that I am fully responsible for any medical costs.
Health Information
I confirm that I am responsible for providing any important information about my child, including allergies, medical conditions, or special needs, before participation.
Photo & Media Consent
I understand that photos or videos may be taken during activities. I give permission for my child’s images to be used for promotional purposes, unless I write in the notes that I decline.
Code of Conduct
I understand that unsafe, aggressive, or disruptive behavior may result in my child being removed from an activity or program without refund.
Acknowledgement
By proceeding, I confirm that I have read, understood, and agreed to this waiver and consent to my child’s participation under these terms.
